Evidence of the monotropic FSH rise, or accelerated follicular development a day 3 estradiol ; . A cycle day 3 FSH and estradiol level is our recommendation as the most simple, straightforward assessment of relative reproductive age. We consider an FSH level of 10 mIU ml to be significantly elevated based on a mean FSH level of 5.9 1.1 mIU ml x SD ; normal women age 20 to 25.10 It must be understood that a woman with decreased ovarian reserve below the critical threshold may have a normal FSH if she is also experiencing accelerated follicular development and the estradiol is elevated on day 3 sufficient to suppress the FSH. We found mean cycle day 3 estradiol levels of 47 range 22 to 78 ; and 54 range 20 to 108 ; pg ml in younger age 20 to 25 ; and 16 older age 40 to 45 ; normal women.10 In the clinical setting, we use 80 pg ml our cut-off for a day 3 estradiol. The relevance and meaning of an elevated day 3 FSH or estradiol is the samedecreased ovarian reserve and relatively poor prognosis for future fertility. The medical literature is quite clear that a woman with elevated day 3 FSH or estradiol will: 1 ; demonstrate decreased response to ovulation inductiona lower estradiol level with fewer follicles and oocytes; 2 ; require a higher dose of gonadotropin; 3 ; have a higher assisted reproductive technologies ART ; cycle cancellation rate; and 4 ; experience a lower pregnancy rate. Certainly the day 3 FSH estradiol level will vary cycle to cycle in any woman, but only rarely less than 10% of the time ; does a woman vary between the normal and the abnormal range when multiple cycles are examined. It has been demonstrated that a woman with a prior day 3 elevation of FSH or estradiol will not respond any better to ovulation induction even if stimulation is withheld until there is a cycle in which these levels are subsequently in the normal range.23 There is a controversy whether day 3 levels of inhibin B are useful in the clinical setting. While a day 3 inhibin B level may help to further define the ovarian reserve, it is not a readily available or useful clinical test at this time. An inhibin B level of 45 pg has been proposed as the critical level.24 However, since the immunoassay ELISA ; for dimeric inhibin B is not readily available outside of research settings, is tricky to run and standardize, and is expensive, we do not currently recommend its routine clinical use. Storage of Medication For safety reasons, keep all medications away from children, pets and other persons. Keep the medication dry until it is mixed. Be sure to use ONLY THE AMOUNT directed. The supply given to you may be enough for the entire treatment period approximately 35 ; or you may be given just enough for a specified number of treatments, in which case, you will need to be resupplied. Be sure to request more medication if your supply is low. If you have pills left over at the time you complete all of your radiation treatments, the unused pills must be returned to the contact person in the radiation therapy department. Once you are started on the supplements, it takes three to five days of being consistent with the dosage, taking the pills on time, for the change to occur in a patient. Once five days have gone by and the changes have not occurred, staying at that dosage longer will not effect more improvement. That is why it is necessary for us to see our patients regularly and make adjustments to the dosages each week until we reach that target dosage for each patient. It is also necessary for the patient to take the supplements on a daily basis and not miss doses. If you miss a dose, your levels of neurotransmitters fall below the desired levels again, and the beneficial effects of taking the supplements are lost for that time period. It takes three to five days, again, to reach maximum benefit. With the drop in a dose, you put yourself back three to five days, again, in reaching the ideal level of your neurotransmitters.

The luteal phase prepares the uterus for implantation. In the natural cycle, it lasts for 11 - 17 days with an average of 14 days. It ends with menstruation. Luteal phase support is given with progesterone pessaries or spaced hCG injections to prevent menstruation occurring in less than 11 days from egg collection as it may do in stimulated cycles. Board Certified, Reproductive Endocrinology and Infertility Infertility is a complex medical, emotional and social condition that afflicts more than four million reproductive age couples in the United States. Successful fertility treatment includes not only achieving pregnancy, but also achieving it in the most efficient and cost effective manner possible. The frequently ignored psychological toll of repeated treatment failures must also be considered. To achieve success, it is imperative that a timely and complete evaluation of both partners be performed. As our knowledge of reproductive physiology has expanded, the fertility workup has evolved as well. In this article, the contemporary fertility workup will be discussed. Attention will also be given to organizing the evaluation to prevent unnecessary testing. Evaluation of the male It is natural for the attention of the gynecologist and family practitioner to initially turn toward the female in cases of infertility. Although infertility is generally viewed as a `female problem', fully 45% of infertile couples have male factor as a contributing cause. It makes sense then to begin the fertility evaluation with a basic evaluation of the male partner. Because significant male factor is generally treated with in vitro fertilization, needless hysterosalpingograms, laparoscopies and clomiphene cycles can be avoided by early detection of significant dysfunction in the male partner. The savings of time and money can be tremendous. In vitro fertilization with intracytoplasmic sperm injection IVF ICSI ; has made it possible to successfully treat virtually all cases of male factor infertility, even with only a few moving sperm in the entire ejaculate. The evaluation of the male partner starts with a competent semen analysis. Nonspecialized laboratories, such as LabCorp and Unilab, perform a World Health Organization WHO ; semen analysis. This is a crude screening test and should be replaced by the strict semen analysis Kruger ; that is done by most fertility centers. The difference between the WHO and the Kruger test is that, with the Kruger test, sperm morphology is evaluated in a very stringent manner. The results of the Kruger test predict fertilization rates in vitro and presumably in vivo as well. The WHO does not predict outcome and will frequently miss subtle but clinically significant sperm abnormalities. The cost of the Kruger test is the same or less than a WHO analysis at our center. Ovarian stimulation with clomiphene citrate CC ; has been used in the treatment of infertility due to ovulatory dysfunction for several decades 1 ; . The combination of CC with timed intrauterine insemination IUI ; has also been found successful in the treatment of unexplained infertility, male subfertility, and endometriosis-related infertility 2 6 ; . Unlike the critical needs in testosterone estimation discussed earlier, there is minimal difference between total and free E2 values in view of the limited binding of the circulating oestradiol to albumin SHBG. 23 ; Therefore, although confirmations by evaluating the nonSHBG oestradiol 24 ; may be interesting, the presently observed age-independent increase in total E2 levels in Group B ED patients who had significantly lower T levels correlating with the history of loss of libido ; needs further understanding. The clinically pertinent E2-T imbalance seen in this data review indicates that elevated oestradiol may play a role in the causation and perpetuation of ED and may determine limited therapeutic responsiveness to T supplements, if they are prescribed for simple correction of loss of libido. It is known that testosterone supplements improve sexual function only in a small group of patients diagnosed with hypogonadism. Given the physiological antagonism of E2 with T, 21 ; the value of administration of T preparations that may be aromatised to E2 in men with the existent state of relative hyperoestrogenism is further questionable. It seems likely that together with the use of currently available oral phosphodiesterase type 5 inhibitors, inclusion of an antioestrogen such as clomlphene citrate and aromatase inhibitors in the standard ED regimen in these patients may provide considerable improvement of the presenting complaints of loss of libido and ED. According to Guay et al, 25 ; empirical clmoiphene citrate improved sexual function in a group of ED patients with hypogonadism and low T levels. However, there was no estimation of E2 levels in that study. Although the aromatase inhibitor, anastrozole, increased both bioavailable and total testosterone and reduced oestradiol ; in a small group of elderly patients in another study, 26 ; there was no concurrent improvement in the IIEF scores, indicating the importance of other compounding coexistent factors. Therefore, taken together, we believe at this stage that further studies with a larger sample size are necessary to substantiate our preliminary clinical hypothesis of the role of E2 in and to translate our experimental evidence of hyperoestrogenism-mediated erectile impairment 7, 8 ; to ED patients. Furthermore, presence of concurrent organic conditions that predispose to similar hormonal derangements such as non-insulin dependent diabetes mellitus 9, 27 ; and and compazine.
